My site's URL (web address) is: http://chipshot.lansa.com/
When searching for my site pages on Google (using site: option) it shows search results snippets that include meta description - but also other useless data. Some of this is fields on the form and other data I am not sure where it came from. See attached file for examples.
Is there a way to ensure only Meta description will show - or exclude garbage from snippet?

Meta Description is used by Google when the MD is useful and representative of the content on the page. When the MD is not useful OR when Google feels their own efforts of extracting information from the page is more beneficial to their customer (searcher) then Google is free to develop their own snippet.
As to your MD, it does not match the content on the page because there is no significant content on the page, therefore Google is going to generate a meaningful snippet relative to the on-page content including the content that you are trying to hide from Google via Java-script.
By the way, having a telephone number of 800.000.0000 and a PO Box for a business address is not a high quality signal. Nor is a processing time of (plus 3-4 business days for processing)

so add meaningless content such as our friend http://lipsum.com/ as a template to be shared across every page & tag within the site.
