Google Panda has given me diabetes. I drank too much sugar the past year ( coca cola ) and had too many worries all at once. Now I am a type 2 advanced diabetic.

That doesn't even phase me. I wouldn't be surprised if High Blood Pressure and Blood sugar is a common trait amongst all SEOs.

1. I see at least three domains where their front page links to yours.
That's page rank passing, or paid links, violating the google TOS.

2. on Jan 5, 2009, your domain was comment spammed to the latimes:

I strongly recommend you do the following:
- start by moving your content to a new domain
- get a new SEO company
- read, understand and follow the google SEO guides and guidelines
- make sure your SEO folks sign off on following the google SEO guide
- do not 301, 302 or meta refresh the old domain to the new domain,
 the penalty will transfer

I only got my hosting provider to renewd my domain for a period maximun of 2 years. Is it possible to renew ti for even 10 years?

How are you going to do that?  authorative backlinks are not built, they are earned.  Sure, it you earn enough authorative backlinks, cease building crappy backlinks and with a little bit of time the impact of the Indian crap will be deminished and in enough time even ignored.  But you don't get to build, acquire, buy, beg, borrow or steal authorative backlinks.
 
That said, check on the authorative backlinks you already have.  Doc's alumin groups (big name Med School, yada, yada, yada) are those links to the proper canonical version of the website?  Are there  journal articles (real journals, you know, peer reviewed NEJM et al) with links to the site and are they current/proper?  What about trade groups and hospital referral services?  In other words tend to your garden of exising links.

start with the SEO-Idiots that you hired and ask for a list of the links they generated for ya.
 
(if you can get a list, it would be a great place to start).
 
 
Dig through Google WMT, MajesticSEO and OpenSiteExplorer.  Yes, all three.  Remember, just because Google doesn't (or does) show the links in WMT doesn't mean the links are good or bad.  The same with the other two.
 
Use Bing/Google to find references to your site.  Using Google and searching for your url I was able to find 37,000 references to the site.
 
Not all of those 37,000 references are bad, not all are real links (show just show the url in text form..... ie a yellow page listing ...... but do not have a real hyperlink attached to the text), but most are real crappy links
 
(see why the list from the SEO-Idiots would be helpful)
 
 
So glove up and dig in.  This is going to be like a fecal impacted 900lb high fiber diet fat woman.  You got to dig in with both hands and dig in some more.
 
(sorry, I have been waiting to use that fecal joke for a long time..... and the time seemed right).

They sent me the reports for the links they created, but the problem was, was that they did it at the end of 2 months so during the process, I had no idea what kind of garbage i was getting into. On one hand it was filthy cheap garbage, but on the other hand, look at my rankings. 

Suppose i have this list of back links they built, how do i proceed from here? How can I de-index them, de-route them from my site, or delete them out of their existence in general.

Huh, both my Blog, and my site http://petfinches.net (And that will probably count as link spam) dropped off the radar Apr 24. I have not done anything that I can tell is against their rules, but visits went from a few to none, and without revenue to even pay the hosting fees, I guess they are history. I think Google is shooting people who have a desire to actually create their "quality content" in the foot. Pretty soon no one will be able to afford it. I had to give up writing for my Energy blog a year ago because I cannot afford the time, and the Finches site hasn't even had a chance - despite plenty of positive feedback from both sites. Unfortunately, Google controls the purse strings, and it is quite apparent, they wish to have Amazon and General Electric as the primary search results.

As far as "Renewing for five years", exactly how can the average person PAY for that? I can't even afford hosting now.

Hey

First off, that really sucks. Is that your own personal site or a client's? 

Getting rid of the backlinks is never easy. You have a few options, one of which is to try to get rid of them or you can switch domains and do 301 redirect to another new domain with a SEO Wordpress Optimized Theme and build that up. That would honestly be the quicker solution if you wanted to get the traffic and rankings back up. Local SEO ranking is pretty easy compared to National SEO as long as spammy links aren't used. Not your fault, it's always tough when working with outsourced vendors. 

P.S. You currently have 2 home pages that I see (not good for SEO - Duplicate Content).

Start with the basics.  If they provided you a list of urls, they might even give you a list of email addys.  Let your fingers start there.  Beyond that, it is doing a bunch of whois and hoping the webmaster is co-operative.
 
 IF there is a common internal page (ie doc's about page) with a bunch of back links then dump the current page so it shows 404..  NO 301 redirect.  NO 301  redirect.  Got that?  NO 301 redirect.  Let the links go to a 404 page.  And then republish the about page (in my example) with a slightly different url name.  Bingo.  Those links are gone.
 
Of course if the links are to home page you are kinda of screwed....... short of starting with a new url (AND NO 301 redirects from the old url).
 
 
Of course tend to your own garden of quality link sources. AND fix any site problems  (such as your 71 broken and redirected links, weak page titles, duplicate meta descriptions and your multiple canonical issues with 22 pages returning a 200 response, the poor speed issues http://gtmetrix.com/reports/entnynj.com/xhHBAzgu with failures to serve scaled images etc.) because these issues drag down the quality score of the site.

I have been told by a Googler with direct knowledge that this will likely work.  His exact phrase was that high quality new links would work to balance the changed algo over time.

He did also mention that it is conceivable that some domains will have such bad link profiles that starting afresh would be better.  Note he said "conceivable".
