I did look at the Legal Issues forms. The defamation form does not have any note fields in it, so there is no place to leave the restraining order details - see http://support.google.com/youtube/bin/request.py?contact_type=defamation .

There is an "other legal issues" form that comes close, I guess - see http://support.google.com/youtube/bin/request.py?contact_type=otherlegal ,

I suppose we could turn in multiple forms - one for those videos with copyright violations, one for the videos with defamation,  and one for videos that abused privacy. We could then add the restraining order specifics in the field for "Please describe how the content in question is allegedly violating this law:"
