"Login with this username/password failed" when creating or linking approved AdSense account

For you folks trying to create a new AdSense account or linking to an existing one in your account, we're aware of the "Login with this username/password failed" message you're receiving. Our engineers are working to resolve this as soon as possible and remember not to submit an application through google.com/adsense unless you have your own website.

TUESDAY 05/29 UPDATE:
For those of you in the US - hope you had a good Memorial Day weekend! Engineering has identified what went wrong, it's being fixed and the longest this may take is a week, but aiming for sooner. I'll keep you all updated, so thanks for your patience.
As a friendly reminder, remember that the majority of you do not need an AdSense account to begin monetizing your videos. You'll need one for a payout once you reach the payout threshold for your country and we'll remind you when it's a good time to link, and there's no time limit, so you'll be able to do this once this issue is resolved or at a later time without being affected.
Also, if you have already associated an AdSense account and want to relink, remember that it's against AdSense's Terms and Conditions to create a second account, and so you should only be using this relinking feature in very exceptional situations such as when you close your account and open a new one because you've moved countries.
