Here's another suggestion which may help to resolve these type of issues, at least in part. Rather than videos being marked as 18+ because someone has flagged them how about giving video makers the option to mark their own videos as suitable for an adult audience only? It seems rather absurd to treat us like children who need to get slapped on the knuckles or sent to the naughty corner for making videos which contain content broadly definable as 'adult' and not give us the opportunity to do so ourselves. I can imagine you will think this would lead to endless adult type videos on site but they're there anyway, with inadequate resources to police them in a sensible and consistent manner. It would seem wise to at least give us all the chance to behave like responsible adults, as we're expected to be everywhere else in society. The way the system is organised now actually encourages childish and vengeful behaviour and leaves you the mess to clear up, which is neither helpful nor cost effective.
