Aha, I changed my password recovery number, and voila, my Google Voice number is now receiving SMS properly.

Resolution: Using your Google Voice number for password recovery breaks SMS reception. 

Googlers, please tap open that bug reward fund, I think I earned this one.

Also, please fix the submission script. When I submitted my first problem report, I was told my messages exceeded the limit of 8192 characters - the actual character count was around 1050.
