Traveling overseas -- how will using Google Voice affect my cellphone bill?

We'll be traveling to Germany and Italy this summer to visit family and friends and would like to be able to call relatives from within one country to another, especially while travelling between countries. When asking around for inexpensive alternatives to using my iPhone overseas (instead of signing up for AT&T's international roaming plan), Google Voice was mentioned quite frequently. I've read the HELP files for GV and am still somewhat unclear about a couple things and was hoping someone with the knowledge or experience would be able to help me.
My questions are (I currently have a GV number):
If I were to make or receive calls or text messages from Italy to Germany (or vice versa) from my iPhone using GV, or to another phone within the same country, how is that call routed and how will it affect my AT&T bill (assuming I'm making or receiving calls from/to landlines)?
How would it be different (if at all) if I made/received calls to/from another GV number instead of a landline?
Any help would be greatly appreciated, including suggestions or answers to questions I may have forgotten to ask ;)
