Please note that Google services millions of people and in doing so they are going to utilize automated processes to evaluate names. (Edit: I am referring to the initial challenge of a name, not a review. Reviews are performed by real people.) As frustrating as it is for users, some names are going to be challenged. In the case of it being your "real" name or a name that you can show proof that you are known by (through links to other sites), you will have the option of providing documentation to support either of those situations.
However, it is better to use the link in your suspension message than to use the one that +Gaurav provided. The form +Gaurav linked to is not the same one that is provided for Google+ profile suspensions. The link to the proper form does not (currently) appear in the first suspension message that tells you that your name is "pending review." Once the review is complete, if your name was not approved, the next suspension message should contain the proper link that you need to use to submit additional documentation.
