PIN entered, tax information submitted, and check send-address entered years ago

No self-hold

No alerts, messages, or holds on the AdSense dashboard but still receiving "Your AdSense payment is on hold" email every month

Now AdSense is saying Next Payment is "To be issued by Jan 31, 2012" instead of December.

Help, please?
